On June 22, 2012, the United States District Court for the Northern District of California granted preliminary approval of the proposed settlement in the Mahbod Khalilpour v. Cellco Partnership d/b/a Verizon Wireless lawsuit. A notice that summarizes the terms of this proposed settlement and important deadlines can be found here. For your convenience, you can either print out and complete the claim form and mail it to Khalilpour Settlement Administrator, P.O. Box 4540, Portland, OR 97208, or, in the alternative, go to www.khalilpoursettlement.com and submit a claim form electronically. You can also call 1–877–868–0171 to request a claim form to be mailed to you free of charge. Make sure to follow all the instructions on the claim form. The claim form must be completed and submitted within 180 days of the court's final approval of the settlement.
