Complete the GTA Service Inquiry Form and fax to 678-868-2083 |
| Step 4 |
A Georgia Government Account Manager will respond within 24 hours to discuss your needs and make recommendations. |
| Step 5 |
When ready to order, complete the following documents:
- GTA Agency Agreement - Affiliates your agency with the statewide contract
- GTA Government Liability Form - Designates those authorized to make purchases
- Execute a Purchase Order for Services & Equipment desired
- Complete the Federal and state tax exemption:
- Federal Tax Exempt Declaration and
- Sales Tax Certificate of Exemption |

Fax all documents INCLUDING a copy of your Agency's Tax Exempt Form to your Verizon Wireless Sales Representative or to 678-868- 2083. |

| After completing the steps above your account will be established and your first order processed. Initial orders may take up to 5 business days to fulfill. Fulfillment on established accounts is 2-3 business days. All equipment is shipped via Federal Express (signature required). |
