Bracken’s company created the customized app--SelectCare Connect—to provide healthcare professionals assistance in preparing patients for discharge from hospitals. The tablets are loaned to the patients enabling them, via the app, to search for skilled nursing facilities in the area for follow up care. Secondary search results find matches for the patients’ clinical and personal needs.
