The PIN Unblocking Key (PUK) is utilized to reset a blocked SIM card.
For Postpaid accounts, you must obtain the PUK via the My Verizon website before proceeding.
For Prepaid accounts, contact customer service to obtain the PUK before proceeding.
Open VZAccess Manager.
From the Windows desktop, click Start > All Programs (Programs) > VZAccess Manager.
With Windows 10, click Start > Apps icon (located in the top-left corner) > All apps (located in the bottom-left corner) > VZAccess Manager.
If presented with Unblock SIM card window, skip to step 4.
Click Options > Preferences.
From the WWAN Device option, click Unblock SIM PIN.
Enter the following information then click OK.
PIN Unblocking Key (PUK)
The 8-digit key obtained from Verizon Wireless.
A PIN code consisting of 4 to 8-digits.
Confirm new PIN
Retype desired PIN code.
If an invalid PUK is entered too many times, the SIM card is permanently disabled.
