The PIN Unblocking Key (PUK) is utilized to reset a blocked SIM card.
For Postpaid accounts, you must obtain the PUK via the My Verizon website before proceeding.
For Prepaid accounts, contact customer service to obtain the PUK before proceeding.
From the Start screen, select the right arrow.
If already prompted to Enter PUK code, skip to step 6.
Select More (three dots located in the lower-right).
From the SIM security section, select change SIM PIN.
Enter the 8-digit PUK obtained from Verizon Wireless then select enter (located to the right, above the numeric keypad).
If an invalid PUK is entered too many times, the SIM card is permanently disabled.
Enter a new SIM PIN (4 to 8-digits) then select enter.
